Oracle Text
Whenever you cast a spell from anywhere other than your hand, you may pay
. If you do, copy that spell and you may choose new targets for the copy. If the copy is a permanent spell, it gains haste and "At the beginning of your end step, sacrifice this permanent." (A copy of a permanent spell becomes a token.)

Any spell you cast will be copied if you pay the cost, not just one that requires targets.
You may only pay the cost once, as the triggered ability resolves. You can't pay it more than once to get additional copies.
If a copy is created, you control the copy. That copy is created on the stack, so it's not "cast." Abilities that trigger when a player casts a spell won't trigger. The copy will then resolve like a normal spell, after players get a chance to cast spells and activate abilities.
